This is usually a tablet but it can be a solution, lotion or gel that you apply directly to your skin. You then have your light therapy in an air-conditioned, walk-in cabinet containing long fluorescent light bulbs. At first, you have a very short session and gradually build up over time to several minutes. Tacrolimus ointment is applied thinly to affected areas of the skin once or twice a day until your skin clears.

Please remember that you should never stop taking steroids (nor reduce the amount you are taking) without your specialist advising you to do so. It’s important to note that losing weight is not recommended if you have an active tumour or are currently receiving treatment.
During flare ups, applying steroid creams help to reduce the inflammation. These are steroids that are taken orally as a pill or injection in the muscle (intramuscularly). Examples are oral prednisolone or intramuscular triamcinolone acetonide (Kenalog). Systemic corticosteroids mimic the effects of hormones your body produces naturally.
You usually start steroid treatment with a high dose that is gradually reduced over 2 to 3 weeks. After a few weeks, you might stop the treatment or you might carry on taking a low dose for a longer period of time. Rather than fighting cancer directly, immunotherapy stimulates your immune system to attack cancer cells. It makes your immune system react to the cancer cells the same way that they would react to an infection of a ‘foreign’ substance, such as bacteria.

high doses it can cause reversible problems to the kidney or liver
function and cause paraesthesia (tingling in hands and feet), fatigue
and headache. The decision to start, stop, continue or change a prescribed medicine before or during pregnancy should be made in consultation with your health care provider. It is very helpful if you can record all your medication taken in pregnancy in your hand held maternity records.
These are steroids that are injected into the affected areas just underneath the skin using a fine needle. Examples of intralesional corticosteroids are hydrocortisone acetate and triamcinolone acetonide. Their effectiveness is limited by their absorption through the scalp.
Treatment is usually continued weekly until the hair has re-grown, which may take up to 12 months to achieve. Long term use of cyclophosphamide is considered undesirable and has
been linked to an increased risk of some types of cancer, eg cancer of
the bladder.
You’ll also need to have regular blood tests while taking immunomodulating agents, particularly in the early stages of treatment. These treatments can cause a number of different side effects, which your specialist can discuss with you. You should also maintain good oral hygiene by cleaning your teeth at least twice a day, and having regular dental check-ups, so that any problems with your teeth or mouth can be identified and treated early. After the papules have cleared, the affected area of skin can sometimes become discoloured.
If your skin is very dry, you may also need to use a greasy emollient. Chlormethine is not absorbed through the skin and does not affect your blood counts, so you do not have to have blood test monitoring during treatment. The effect of desensitisation treatment wears off in the winter, so it should be repeated every spring. PLE is a response to sunlight and as little as 20 minutes of sun can trigger the problem. PLE can come up even when the light has passed through window glass and sometimes even after exposure to fluorescent lighting. Both long (UVA) and short (UVB) wavelengths of ultraviolet light can cause PLE, but it is still not clear how they trigger the rash.
They can also be used alongside chemotherapy treatment – for example, as part of a combination of drugs known as ‘CHOP’ or ‘CVP’.
